When You Arrive:

• 1) Check in at the front desk and meet the friendly and caring hygienist   
• 2) Have your dental insurance information with you
• 3) Be prepared to fill out health history, hygiene treatment consent and privacy forms (HIPAA) for 

            yourself and all family members who have an appointment

You should arrive 15 minutes BEFORE your hygiene reservation to complete your health insurance forms.

Who is Eligible:
You must not have a dental home or been seen by a dentist within a year. We strive to provide all people with access to dental hygiene care, especially those without insurance or low cost health insurance.
